Initial Application for a NJFP ID Card
An official NJ Firearms Purchaser ID Card must be presented to purchase any rifle or shotgun in NJ. The ID Card must be applied for using NJ Form STS-33 (Rev 11/03) via the applicant's local NJ municipality of residence.
The best procedure is to phone ahead and determine which officer handles these applications and arrange to meet that officer when he is on duty at your local police department in NJ.
This same application form is required for a current card holder to apply for a permit to purchase a handgun. The application may be used to apply for both simultaneously...and for multiple handguns.
The usual disqualification questions are asked and the applicant must sign.
Out-of-State residents may apply for and be issued a NJ Firearms Purchaser ID Card using the same form but may ONLY apply at a NJ State Police Barracks that is not on a toll road. Again, phone ahead to be sure that an officer who knows about application by out-of-staters is on duty and available.
A non-refundable $5.00 fee is required for an ID Card and/or $2.00 for each handgun purchase permit...payable to Superintendent of State Police or to the Chief of Police in the municipality in which the applicant resides and must accompany the application. Application in person may be required and fingerprinting will be required before the ID Card is actually issued. No mailing information is given on the application.
Application for a DUPLICATE of an already issued NJFP ID Card
NJ Form STS-3 (Rev 1/00) is needed to apply for the reissuing of an already issued NJFP ID Card if it was stolen, lost or mutilated. Additionally, if the issuee has changed his NAME, ADDRESS or SEX, a new card must be applied for and issued. At the time of purchase of a firearm...the purchaser is usually asked for both a NJFP ID Card and his current Driver's License...the data on both must match.
If a change is applied for and the original card is in the possession of the issuee, it must be surrendered to be destroyed. Duplicates for stolen or lost cards are marked "Duplicate."
Any card holder who wishes to continue to hold this credential and who moves out of New Jersey must apply for a duplicate at any NJ State Police Barracks that is not on a toll road.
No fee is mentioned on the application form for a Duplicate ID Card and application in person may be required and fingerprinting will be required before the ID Card is actually issued.
The only mailing address that is given on the application is: Superintendent of State Police, S&TS Section, Box 7068, West Trenton, New Jersey 08628-0068...but mailing is NOT indicated as a valid method of application.

If you download a form, the proper number of forms must be printed and signed to be submitted...two for an original application and three for an application for a duplicate card.
Theoretically, photocopies would work as well as downloaded forms if they are clear and clean.
